When Booking a Hospital, Nursing Homes & Care Facilities Notary Service at
Wesley Chapel, Florida
At Rey Ethan Mobile Notary Services, we understand that notarizations in medical or care settings require both sensitivity and professionalism. To ensure a smooth and respectful signing experience, please review the following important information before scheduling your appointment.
Before the Appointment
- The signer must be coherent and willing.
The patient or resident must be alert, aware, and willing to sign. If the signer appears confused, sedated, or unwilling, the Notary cannot complete the notarization.
- Discuss the documents in advance.
Before the Notary arrives, please review the purpose of the document with the patient. Notaries are not attorneys and cannot explain or provide legal advice about the contents of any document.
Witness & Document Preparation
- Witness requirements.
Some documents may require one or more witnesses. Please confirm with the facility whether staff are allowed to serve in that capacity, as many facilities do not permit it. If needed, the Notary can help arrange independent witnesses for an additional fee.
- Have documents ready.
Notaries cannot draft or create legal documents such as Wills, Advance Healthcare Directives, or Affidavits unless they are also licensed legal professionals. Please have all documents completed and ready prior to the appointment.
After the Appointment
- Document return options.
If you cannot be present for the signing, please coordinate with the Notary regarding how you would like the completed documents returned — via UPS, FedEx, or regular mail. Additional shipping or handling fees may apply.

Frequently Asked General Hospital Notary Questions
- Usually, yes. Many facilities control visitor access and often don't allow staff to act as witnesses. Coordination with the facility's unit/administration will be needed in advance to find a convenient time and confirm whether outside witnesses are needed. Rey Etan Mobile Notary Services can help you arrange outside witnesses if required. Reference: Facility/witness policy examples and notary tips. nationalnotary.org
- I speak privately with the signer, ask simple questions (name, date, reason for the document) to evaluate comprehension, ensure there’s no evident coercion, and confirm the signer's willingness. Florida law requires the signer to appear personally and provide satisfactory evidence of identity or be known by the notary. If doubt remains, we reschedule—the signer’s protection comes first. Reference: NNA guidance on capacity and when not to notarize. nationalnotary.org
- No, medical/hospital bracelets and insurance cards aren't valid notary ID. The patient needs a government-issued photo ID. If that's a challenge, I'll explain lawful alternatives such as "two credible witnesses" (in Florida) available before we schedule. Reference: NNA on correctly identifying patients. nationalnotary.org
- Some states allow notaries to act as witnesses; others don't, and many hospitals and care facilities forbid staff from serving as witnesses. I'll verify your document's witness rules and, if needed, bring independent witnesses. Reference: NNA on notary-as-witness; facility policies forbidding staff witnesses. nationalnotary.org
- Often yes. Many states allow a "signature by mark" (like an X) or alternative methods with two disinterested witnesses. Florida is one of those states where, if the signer cannot physically sign but can appear and understand the document. I'll ensure the wording is correct and that the setup is in compliance with Florida law. Reference: Governor's Reference Manual for Notaries. flgov.com
- No. HIPAA doesn't require notarization or witnesses. Some families choose notarization for extra assurance of identity and consent. Reference: Does a HIPAA Authorization Need to Be Notarized? askfeather.com
- Have the document fully filled out but unsigned, the patient’s valid ID, required witnesses, and a quiet time window (coordinate around hospital tests/rounds). I’ll handle the journal, certificate, and any needed oaths.
Reference: Notary medical-signing prep pointers. nationalnotary.org,
- Yes, many of my mobile notary work involves Florida POAs and Advance Directives in hospitals/rehabs. Provided the signer is competent, willing, and present. Some skilled nursing facilities require an ombudsman for certain directives nd we’ll work it out ahead of time.
- It depends—but plan for a little longer than a standard office notarization. We may coordinate with rounds, visitor limitations, or staff escorts. I book enough time so we can complete the signing without stress.
- If a patient appears confused, overwhelmed by medication, or under pressure, I must pause or decline the notarization. Florida law requires presence and willing signing. In those situations, I'll recommend we reschedule when the signer is clear, or you may consult counsel. Reference: NNA on capacity and refusal. nationalnotary.org
